This used to happen lots in the editor, but seems to have gone away. Game UIs should be explicit about setting an initial focus when transitioning between screens.

This often happens if I run a game from within the editor, then exit the game. Focus seems to land somewhere outside of a control and can't be restored, and my defensive code doesn't kick in.
